BASIC REQUIREMENTS

Various institutions offer financial assistance to cancer patients. While there is some variation in requirements per agency or organization, in general, patients must prepare the following documents:

  1. Valid ID 
  2. Medical/Clinical Abstract with hospital logo and your doctor’s complete name, signature, and PRC license number
  3. Treatment Protocol
  4. Laboratory/Procedure Order/Request
  5. Quotation 
  6. Certificate of Indigency from your barangay
  7. If patient is being assisted by a guardian, the guardian must also present a valid ID and an authorization letter

GOVERNMENT PROGRAMS

Malasakit Center

The Malasakit Center is a one-stop-shop located within selected government hospitals  that helps poor and indigent patients access financial assistance from Philippine Health Insurance Corp. (PhilHealth), Philippine Charity Sweepstakes Office (PCSO), Department of Social Welfare and Development (DSWD), and the Department of Health (DoH). 

Use of Malasakit Centers is free of charge and assistance is given in the form of a Guarantee Letter (GL). The centers do not provide direct cash assistance. They are usually located near the billing section. 

Steps:

  1. Patient (or authorized representative) submits request for assistance
  2. Patient fills out Unified Form
  3. Patient provides requirements:
    1. Billing
    2. Medical abstract
  4. Malasakit Center personnel (social worker) interviews patient
  5. Malasakit Center submits the request to participating agencies.
  6. Participating agencies process the request.
  7. Upon approval, the participating agency releases assistance in the form of a guarantee letter. 
  8. Malasakit Center personnel forward the approved amount to the hospital billing section.

Important reminder
To avail of Philhealth benefits, member must have at least three (3) months’ premium contributions within the immediate six (6) months prior to the month of availment.

For Working/Self-Employed Women

To enable the hospital to deduct PhilHealth benefits from the hospital bill, fill out PhilHealth Claim Form 1 and submit to the hospital billing section prior to discharge of the patient. Submit official receipts of medicines bought outside the hospital during confinement for possible refund. The hospital and doctor/s are supposed to deduct the benefits from the total bill before payment. If you pay the full amount for professional fees (that is, the doctor does not deduct any PhilHealth benefits), ask for a Waiver and attach doctor’s Waiver and the official receipt for professional fee to Claim Form 1. Submit also your Member Data Record (MDR) to support your Claim Form.

If you cannot submit a properly filled out Claim Form 1 and MDR, the hospital will not deduct PhilHealth benefits. In this case, pay the hospital bill in full and file your claim directly with PhilHealth for a refund of payments. Submit the following:

  1. PhilHealth Claim Form 1
  2. PhilHealth Claim Form 2
  3. PhilHealth Claim Form 3
  4. Hospital Waiver
  5. Physician’s Waiver (if full and actual professional fee was paid by patient)
  6. Official receipts of medicines bought outside the hospital during confinement

The claim must be filed with PhilHealth within 60 calendar days from discharge of the patient from the hospital, or within 60 calendar days from 1st treatment (first day of radiotherapy, first day of chemotherapy). Claims may be filed at the PhilHealth branch office nearest the member’s residence or place of work.

Philhealth’s Z Benefits for Catastrophic Illness

The Z Package is a P100,000 benefit offered to  Philhealth members and their qualified dependents who are  diagnosed as stage 0 to IIIA breast cancer patients. To qualify, it is IMPORTANT to go ONLY to the selected contracted hospitals (listed below) and have the procedures done from the start of the diagnosis to the end of the treatment. Coverage for breast cancer is P100,000. If total cost is greater that P100,000, Philhealth member will co-pay.

PCSO’s Medical Assistance Program (MAP)  issues guarantee letters to partner hospitals and covers the cost of medicines, chemo drugs, radiation therapy, and other medical needs of patients in those institutions. Access the Online Application Form here.

Procedure on How to Avail of Medical/Financial Assistance through NCR-MAP Online Application

  1. Prepare all the documentary requirements
  2. Scan all the documentary requirements
  3. Go to the official PCSO website at www.pcso.gov.ph, click E-Services and click NCR Online Application for Medical Assistance to proceed with the application
  4. Fill-in the required information and details of the patient
  5. Upload the scanned documents to the system
  6. Wait for the notification of the status of the request to your email within 24 hours. If approved, print the Guarantee Letter (GL) and the MAP application form
  7. Submit the printed Guarantee Letter and MAP application form including the complete documentary requirements to the Hospital, Dialysis Center, partner health facility (PHF)/medicine retailer or Diagnostic Center
  8. A notification on the status of the request via email shall be provided within 24 hours from the submission of application and complete documents

The MAP NCR Online application is open from Monday to Friday from 8:00 am in the morning until they reach the allowable number of cases for the day. In case application did not proceed, may re-apply on the next working/business day.

DSWD Assistance to Individuals in Crisis Situation (AICS)

DSWD issues guarantee letters for chemotherapy medicine through its Protective Services Program for Medical Assistance

How to avail:

e. Social Case Study Report from the City/Municipal Social Welfare and Development Officer (For hospital Bill)
f. Quotation from Globo Asiatico. Call Globo Customer Care (info below). They will ask for a copy of your prescription, then issue a quotation to be picked up from their office. This makes the process faster than if you only provide DSWD with a prescription. (TIP: Schedule pick up on the way to DSWD Main as they are both in Quezon City).

2. SUBMIT DOCUMENTS TO DSWD CRISIS INTERVENTION UNIT.
Preferably the Main Office near Batasan Complex in QC. You can also go to a satellite office but better if you can go to Main)
a. Get a number from the guard. You will be given a short form to fill out.
b. Document checking. A social worker will review your documents. If they are complete, you will move on to the next step.
c. Interview. A social worker will ask some questions to evaluate your situation. After that, he/she will put in the request for a Guarantee Letter (GL). The amounts range from 5,000-75,000.  (Be prepared to spend half a day at DSWD)

3. RELEASE OF GUARANTEE LETTER.
If your application is approved, your social worker will text you to pick up your GL from DSWD within 1-4 business days.

4. DELIVER GL to Globo Asiatico. (Address provided above. This can be done directly from DSWD).

5. WAIT.* Globo and DSWD will coordinate for the payment of your meds.

6. RELEASE OF MEDICINE FROM GLOBO.*
*Please note that even though the application and approval process is fast, the actual RELEASE of the medicine can take long, from weeks to months. If you are on meds now that you’ll need to continue for the next few months, it is best to start applying for this ASAP.

 

DOH Cancer Support and Palliative Medicine Access Program (CSPMAP)

The Department of Health (DoH) provides free chemotherapy to various cancer patients, including breast cancer patients diagnosed as Stage 0, Stage 1, Stage 2, Stage 3A, and Stage 3B. It has an “Alay Chemo sa CI” program. Assistance is given only to indigent patients (Class D) following evaluation of the Medical Social Services (MSS). The assistance depends on the need and may be given as full or partial support for chemotherapy drugs. The financial support is only for chemotherapy, antibiotics, antiemetic, and other medicines needed by the patient while undergoing chemotherapy.

The PGH Cancer Institute administration office is open from Monday to Friday, 8:00 am to 4:30 pm.

Sickness Benefits

To avail of sickness benefits, you must notify the SSS or your employer within five (5) days from the day you miss work or are confined to your home. However, this does not apply when you are confined in a hospital.

For hospital confinement, the claim for benefit must be filed within one (1) year from the last day of confinement from the hospital. For home confinement, the claim for reimbursement by the employer must be filed within one (1) year from the start of illness. Failure to file the claim within the prescribed period will result in denial of the claim.

What forms are needed in filing for sickness benefit?

  1. For the employed member: SSS Form CLD-9N (Sickness Notification).
  2. For the employer: SSS Form B-304 (Sickness Benefit Reimbursement Application)
  3. For the unemployed/self-employed/voluntary member
    1. SSS Form CLD-9A (Sickness Benefit Claim for Unemployed/Self-employed/ Voluntary Members)
    2. SSS Form MMD-102 (Medical Certificate)
  4. Other documents:
    1. For unemployed members
      • Certification from last employer showing the effective date of separation from employment or notice of the company’s closure/strike or certification from the Department of Labor and Employment that the employee or employer has a pending labor case.
      • Certification that no advance payment was granted, if the date of separation from employment is within the confinement period being applied for.
      • Presentation of the SS Digitized ID/SSS Form E-6 (acknowledgment stub) with 2 valid IDs, one of which with the recent photo. To ensure receipt of benefits by members, authorized company representatives who file sickness benefit claims shall present the members’ SS digitized ID or E-6 (acknowledgement stub) with two valid IDs (at least one with photo). This requirement is in addition to the presentation by the company representative’s own SSS digitized ID and blue card.

Where does the member file his claim for sickness benefit?

  1. For the employed/separated member – Claims may be filed at the nearest SSS office. However, processing will be done at the branch where the employer and employee records are based.

Disability Claims

Losing one or both breasts is a partial disability.

The prescriptive period in the filing of disability benefit claim is ten (10) years from the date of occurrence of disability.

What are the forms needed in filing for the disability benefit?

  1. Disability Claim Application (SSS Form DDR-1)
  2. Medical Certificate (SSS Form MMD-102)
  3. Other documents that may be required to support the disability claim such as clinical and laboratory tests results (pathology report) and hospital records.
  4. SSS digitized ID or E-6 (acknowledgement stub) with two valid IDs, one of which with recent photo.

Where can a member file his disability benefit application?

Applications forms of disability benefits are filed at the nearest SSS branch or representative office.
You will be called in for a physical examination by an SSS doctor. This can be done on the same day of submission of documents if the doctor is available at the time of your visit.

ICANSERVE Foundation has forged a partnership with Detoxicare Philippines, a pharmaceutical company that imports, distributes and sells medicines called KAPATIRAN PARA SA KANSER.

Detoxicare will be giving the following drugs, free of charge to breast cancer patients who qualify. The medicines to be donated are the following:
1. Anastrozole (Anastro-cell)
2. Letrozole (Letrostad)
3. Docetaxel (Doxestad) 20mg
4. Docetaxel (Doxestad) 80mg

Who Qualifies?
1. Top priority will be given to indigent patients
2. Patients from the Middle Class whose funds are depleted

1. Upon submission of requirements to the ICANSERVE Foundation, a medical doctor accredited by the Kapatiran sa Kanser program will interview the patient, verify if medicines prescribed are appropriate, based on records to be presented. No additional tests will be requested.
2. Once qualified, the medicine shall be given to the patient. A patient can only avail once and can only avail of one drug from the list.
3. This program is open to anyone in the Philippines but note that medical Doctors who can accredit for the Kapatiran sa Kanser program are based in : Metro Manila, Valenzuela City, Cebu, Davao, Cagayan De Oro, Sta Cruz, Laguna, Bacolod, Pampanga, Pasay, Dagupan. The applicant patient should be able to travel on her own expense to any of these sites for the interview.
4. The partner doctors of the Kapatiran Para sa Kanser program are there for the interview and verification only. Patients are not obliged to make them their regular medical doctor. It’s assumed that patients applying for this program, already have their own health care provider.

Soroptimist International of Ortigas and Environs

Soroptimist International of Ortigas and Environs offers free chemotherapy, as well as hormonal therapy (Tamoxifen), for indigent Stage 1 and Stage 2 breast cancer patients up to 6 cycles. Patients outside the National Capital Region may apply.

Please submit the following:

  1. Complete patient details (name, postal address, landline, cell phone number)
  2. Histopathology report, Chest X-ray and Liver Ultrasound
  3. List of prescribed medicines (chemotherapy agents)

Tzu Chi Foundation provides medical/financial support to patients with Stage I-II breast cancer.
